You can call our office Monday through Thursday between 8am & 5pm (we are closed for lunch from 12-1pm), or we have made requesting an appointment a simple process via our website at any time. Our online appointment requests are for cleanings with dental hygienists only. If you need to see Dr. Warner for an emergency, or would like to schedule a Med Spa appointment, please call our office directly. Our Patient Coordinator will send you a courtesy reminder for your reservation with us 1-2 days prior to your appointment.
If you are unable to make your appointment, kindly give 24-48hrs notice to avoid a $50 per scheduled hour broken appointment fee. If we do not have an appointment time that is convenient for you, we can put you on our short call list and contact you if and when a desired appointment becomes available. We understand that your time is valuable, as is ours.
Insurance and Billing
We accept most dental insurance plans as a non-contracted provider. As a courtesy, we are happy to electronically file your claim for you.
We must have a copy of your dental (not medical) insurance card, the subscriber’s ID number or Social Security number, and the subscribers date of birth in order to file your claim. Please provide this information BEFORE the day of your appointment if possible.
Please prepared to make any deductible payment or copayment at the time of your appointment. Your dental insurance may or may not assist you with cosmetic and/or elective procedures, so be prepared to pay for these treatments in full on the date of service.
We suggest that you call your insurer to verify eligibility and benefits before your appointment if you are unsure of your dental coverage. We are also happy to answer any insurance questions that you may have, so do not hesitate to reach out to us!
We accept checks, cash, or credit cards. We file insurance for your reimbursement as long as your insurance carrier allows you to choose your own dentist.
